Payment Responsibility

 

Payment is due at the time of service unless prior arrangements have been made.

 

Patients are responsible for:

 

• Co-pays

• Deductibles

• Co-insurance

• Non-covered services

• Services denied by insurance

 

We accept major credit cards, debit cards, and approved payment methods.

Self-Pay & Transparency

For patients without insurance, we offer transparent self-pay pricing and are happy to discuss estimates prior to treatment.

 

Our goal is to make care accessible while maintaining fairness and sustainability for our practice.

Insurance Billing

 

As a courtesy, we submit claims to participating insurance carriers on your behalf. However:

 

• Insurance coverage is determined solely by your provider

• Verification of benefits is not a guarantee of payment

• Patients remain financially responsible for all balances

 

If your insurance denies coverage, the balance becomes the patient’s responsibility.

Cosmetic & Aesthetic Services

All cosmetic procedures, aesthetic treatments, and elective services are considered self-pay and are not billable to insurance.

 

Full payment is required prior to or at the time of service for cosmetic treatments.

• A valid card may be required to hold appointments

• Late cancellations or no-shows may be subject to a fee

• Deposits for certain procedures are non-refundable
